Most opening guides focus on the build out and the brand. This one covers the work behind the front desk. It isn't a full legal ownership guide. For that, talk with healthcare counsel, and watch for our upcoming piece on med spa ownership. For the bigger picture on adding services, see adding peptides and wellness services to a med spa.
What do I need to open a med spa?
Beyond the space and equipment, here's the operations checklist we see owners miss most.
1. Medical oversight that's real
- A physician, or the oversight structure your state requires, with relevant training
- Written collaborative practice or supervisory agreements naming each NP and PA
- A chart review schedule everyone knows about
- An adverse event and escalation plan, posted where staff can find it
In North Carolina, the Medical Board expects supervision to be adequate and documented, and ownership rules apply. See who can be a medical director for a med spa or peptide program.
2. Licensed sourcing, decided before you open
- One clear answer to "where do our products come from?"
- Licensed, vetted pharmacy and laboratory partners only, no research use only products
- Prescription products dispensed only against a valid prescription or order from your licensed provider
- A short sourcing policy staff can read in five minutes
3. Scripts for every role
- Front desk: booking, intake, payment, and a polite handoff for clinical questions ("That's a great question for the provider.")
- Clinical team: consent conversations and follow up scheduling, approved by your medical director
- Marketing: describe services, not results. The FTC expects health claims to be backed by competent and reliable scientific evidence.
4. Intake, consent and documentation
- Intake forms and consent documents reviewed by your medical director and counsel
- A chart template for each service
- A plan for how records are stored and who can access them
5. Payment setup
- Cash pay services, with clear policies for deposits, cancellations and refunds
- No language suggesting services are covered by insurance
6. Supply, storage and reordering
- A starting list of what you'll stock for each service
- Par levels and a reorder trigger for each item
- Storage that matches each product's label, including refrigeration where required
- One person who owns receiving, logging and reordering
7. A soft open before any marketing
- A small group of real appointments to test the full flow, from booking to follow up
- A short debrief after each day: what broke, what was confusing, what to fix
- A go or no go check before you turn on ads or social posts
How to open a med spa as a nurse
Nurses bring a lot to a med spa: clinical judgment, patient trust and a feel for how care should flow. If you're an RN or NP thinking about opening your own place, two things matter most.
First, ownership and scope. In North Carolina, businesses that practice medicine generally have to be owned by Medical Board licensees or permitted combinations, and NPs practice under collaborative practice agreements with a physician. That shapes what structure is open to you. Talk with healthcare counsel early, before you sign a lease.
Second, the checklist above still applies. Nurse owned or not, the clinics that launch smoothly are the ones with oversight, sourcing, scripts and supply written down first.
The step owners skip most
If we had to pick one, it's the soft open. It's tempting to launch with a promotion on day one. A quiet first couple of weeks lets your team find the gaps in intake, consent, payment and reordering while the stakes are low. Our weekly operations guide for med spas picks up from there.
Where does RevMed fit?
Building every policy, script and supply process from scratch probably isn't the best use of your time while you're opening. RevMed's launch team helps clinics set up the operational side, including workflows, training and documents, with a soft open before marketing. Ordering runs through RevMed's licensed, vetted pharmacy and laboratory partners, so your team has one place to reorder.
We're based in Raleigh, your local contact for questions and introductions across the Triangle.
The plan:
- Open your clinic account through our Partner Link.
- Launch with RevMed's setup support.
- Order from one source.
Common questions
What licenses do I need to open a med spa?
It depends on your state, your services and your ownership structure. In North Carolina, start with the NC Medical Board's resources and talk with healthcare counsel.
Can a nurse open a med spa?
Possibly, depending on state ownership rules and how medical services are supervised. In NC, ownership and collaborative practice rules both apply, so get counsel's view before you commit.
Should I add peptides at opening?
Many owners start with a smaller menu and add services once operations are steady. If you add peptides, plan oversight, sourcing and staff scripts first.
Do I need a soft open?
We recommend one. It's the easiest way to catch problems before marketing brings in volume.
Where should my products come from?
Licensed, vetted pharmacy and laboratory partners. Avoid research use only products.
